چکیده
UNLABELLED Trans-scaphoid perilunate fracture-dislocation is the most common variety of fracture-dislocations of the wrist. Because of the carpal scaphoid fracture association, post-reduction stability is poor, often requiring surgery. Of the several surgical techniques used over time, percutaneous screw fixation for scaphoid fracture is currently preferred. MATERIAL AND METHODS Six cases of trans-scaphoid perilunate fracture-dislocation were surgically treated by percutaneous reduction of the scaphoid fracture and percutaneous insertion of Herbert screw in a proximal to distal manner. Postoperative immobilization was followed by rapid resumption of wrist mobility. RESULTS Four of the six patients have resumed normal, painless mobility of the wrist within 3 to 6 weeks post immobilization; in the remaining two cases there were a slight wrist stiffness attributed to persistent slight rotatory deficiency between the fractured scaphoid fragments. CONCLUSIONS The percutaneous Herbert screw fixation provides a firm fixation with good compaction of scaphoid fracture, enough stability to allow minimal soft tissue and local damage, early mobilization following minimal immobilization, and thus rapid healing.

Background: Trans-scaphoid perilunate fracture-dislocation and perilunate dislocations are among uncommon injuries, most commonly seen in young patients due to high energy trauma. The treatment can be achieved either surgically by open reduc tion and internal fixation or closed reduction and casting. Methods: To compare surgical versus non-operative results of treatment of trans-scaphoid pe...
متن کاملTrans-radial styloid, trans-scaphoid, trans-triquetral perilunate dislocation.
A rare case of trans-radial styloid, trans-scaphoid, trans-triquetral perilunate dislocation is described. The injury was treated by reduction and internal fixation using a Herbert screw and Kirschner wire. A good functional result was achieved. A review of the literature illustrates that early surgery is recommended. Alternative surgical approaches are discussed.

AbsractAll perilunate fracture-dislocations combine bone avulsions, ligament disruptionand fractures in different forms. The most frequent pattern is the dorsal transscaphoidperilunate fracture-dislocation. The reported risk of missed initial diagnosisin perilunate dislocations is as high as 20%. Concerning pathomechanics of theperilunate fracture-dislocation, it should be noted that most dorsa...
متن کاملAn unusual variant of perilunate fracture dislocations
Trans-scaphoid, trans-radial styloid, trans-triquetral perilunate fracture dislocations are rare. We describe a 19-year-old male who suffered this injury after crashing his bicycle. He underwent open reduction internal fixation and percutaneous pinning. Scaphoid union was achieved at 8 weeks. Near complete range of painless motion was achieved by 4 months.
